Please fill in the appropriate information - be sure to give Category, Title, Author and Item number - then print out the order form, and mail along with payment to the address indicated below payable to Jan Moncrief - including applicable sales tax, total cost of book order and insurance fee (see below for insurance costs)*. Be sure to include the shipping address if different from the address given with the name. Please include your telephone number; we will not call unless there is a question about your order. All orders are payable in US funds and will be shipped within 10 days from receipt of payment. Money orders, cashiers checks, and personal checks accepted. Please allow 7 days for personal checks to clear. For rates on delivery out of the country, please e-mail.
